We identified this as a hardware failure and have now replaced the relevant piece of equipment. We cited critical quotes from an internal email but, in fact, these did not relate to the conference decision. Additionally we misattributed Facebook comments to campaigner Ben Hart and incorrectly said he had attended the conference.

We accept that Mr Mzimba has never worked on such a documentary, did not behave in a drunk or inappropriate manner and was not humiliated as claimed.
